What you will do

• Determine and coordinate required rehabilitation assessments to clarify participant needs.
• Develop goal-oriented Rehabilitation Plans in consultation with participants, families, and Veterans Affairs Canada (VAC) Case Managers.
• Act as a service integrator, collaborating with medical, psycho-social, and vocational rehabilitation professionals.
• Empower participants to identify and utilize community-based interventions and resources.
• Identify risk factors contributing to suboptimal outcomes and implement proactive strategies.

Requirements

• Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, or Human Services.
• Health professional designation or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).
• Minimum of 3 years experience in direct provision of medical, psycho-social, or vocational rehabilitation services.
• Experience working with persons living with mental or physical health problems creating life barriers.
• Ability to obtain federal security clearance (Reliability Status).
• Must be based in Ontario and have the ability to travel across the region as required.

Nice to have

• Bilingual proficiency in French and English.
• Experience working with employers, training institutions, and community resources.
• Current or former members of the Canadian Armed Forces (CAF).
