What you will do

• Determine required rehabilitation assessments to clarify participant needs and coordinate services.
• Develop thorough Rehabilitation Plans in consultation with participants, families, and Case Managers.
• Act as a service integrator, collaborating with medical, psycho-social, and vocational professionals.
• Empower participants to problem-solve through community-based interventions and resources.
• Identify risk factors contributing to suboptimal outcomes and implement pro-active strategies.

Requirements

• Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, or a related field.
• Health professional designation or Certified Vocational Rehabilitation Professional (CVRP/CVRP-F) certification.
• Minimum of 3 years of experience in medical, psycho-social, or vocational rehabilitation services.
• Experience working with individuals facing mental or physical health barriers.
• Employment conditional on obtaining federal security clearance – Reliability Status.
• Ability to travel across the region as required.

Nice to have

• Bilingualism in French and English.
• Experience working with employers, community resources, and training institutions.
• Current or former members of the Canadian Armed Forces (CAF).
