What you will do

• Coordinate and determine required rehabilitation assessments to clarify participant needs and capacity for improvement.
• Develop goal-oriented Rehabilitation Plans in consultation with participants, families, and Case Managers.
• Act as a service integrator, collaborating with medical, psycho-social, and vocational professionals.
• Empower participants to utilize community-based interventions and resources to achieve outcomes.
• Identify risk factors contributing to suboptimal outcomes and implement proactive strategies.

Requirements

• Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, or Human Services.
• Health professional designation or Certified Vocational Rehabilitation Professional (CVRP/CVRP-F) certification.
• Minimum of 3 years experience in the direct provision of medical, psycho-social, or vocational rehabilitation services.
• Experience working with individuals facing mental or physical health barriers.
• Must obtain federal security clearance (Reliability Status).
• Ability to travel across the Atlantic Canada region as required.

• Bilingual proficiency in French and English.
