What you will do

• Determine and coordinate necessary rehabilitation assessments to clarify participant needs and capacity for improvement.
• Develop thorough, goal-oriented Rehabilitation Plans in consultation with participants, families, and case managers.
• Act as a service integrator, collaborating with medical, psycho-social, and vocational professionals.
• Empower participants to utilize community-based interventions and resources to achieve desired outcomes.
• Identify risk factors contributing to suboptimal outcomes and implement proactive mitigation strategies.

Requirements

• Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, or a related field.
• Health professional designation or Certified Vocational Rehabilitation Professional (CVRP/CVRP-F) certification.
• Minimum of 3 years of experience providing medical, psycho-social, or vocational rehabilitation services.
• Experience working with individuals facing mental or physical health barriers.
• Must obtain federal security clearance (Reliability Status).
• Must be based in Ontario and have the ability to travel across the region.

Nice to have

• Bilingual proficiency in French and English.
• Experience collaborating with employers, training institutions, and community resources.
• Background as a current or former member of the Canadian Armed Forces (CAF).
