What you will do
- Conduct virtual primary care visits via telehealth, including comprehensive assessments and acute care interventions.
- Diagnose and manage chronic conditions such as hypertension, diabetes, and COPD with individualized care plans.
- Order and interpret diagnostic tests and coordinate necessary follow-up care.
- Manage medication prescriptions, titrations, and refills while ensuring patient safety.
- Provide health coaching and patient education to empower patients in managing their own health.
- Collaborate with multidisciplinary teams and provide feedback to improve the electronic health platform.
Requirements
- Must be based in the USA.
- Bilingual proficiency in Spanish is required.
- Active RN and APRN or Physician Assistant license in good standing.
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- Minimum 2 years of experience in primary care, urgent care, or internal medicine.
- Active BLS certification at time of hire.
Nice to have
- Experience in telehealth or virtual care settings.
- Familiarity with value-based care models and quality metrics (HEDIS, STAR ratings).
- Proficiency with EHRs and virtual care workflows.
