Case Manager

Salary: $22.00/hour + Bonus Structure

What you will do

• Conduct regular drug abuse case management sessions and patient orientations.
• Collect and assess social, educational, and vocational information for treatment planning.
• Develop and revise patient treatment plans while maintaining accurate records.
• Perform crisis intervention and inter-agency documentation.
• Coordinate care with community providers and facilitate patient referrals.
• Participate in community relations and ensure adherence to regulatory requirements.

Requirements

• Bachelors degree in a related Human Services field required.
• Must be able to work onsite in Westerly, RI.
• Knowledge of addiction treatment methods and current trends in opioid maintenance.
• Ability to handle high-stress situations and interact with diverse patient populations.
• Compliance with federal and state certification and licensure regulations.
